Position Overview

We are seeking a compassionate and skilled Licensed Practical Nurse (LPN) to join our team at Cranberry Park at ShorePointe. The Wellness Nurse will provide direct nursing care to residents in our Assisted Living and Memory Care communities, ensuring their health, safety, and well-being through comprehensive nursing services and a person-centered approach.
Key Responsibilities

• Provide direct nursing care to residents in Assisted Living and Memory Care settings.
• Administer medications and treatments in accordance with physician orders and state regulations.
• Perform resident assessments, monitor health status, and promptly report any changes in condition.
• Assist with the development, implementation, and updating of individualized care plans.
• Monitor and manage chronic conditions common in geriatric and memory care populations.
• Document nursing care, medication administration, and resident observations accurately and in a timely manner.
• Respond to medical emergencies and follow established protocols.
• Communicate effectively with residents, families, physicians, pharmacies, and interdisciplinary team members.
• Supervise and support caregiving staff; provide guidance related to resident care needs.
• Ensure compliance with HIPAA, resident rights, infection control, and safety standards.
• Participate in wellness programs, resident education, and health promotion activities.
• Assist with coordination of physician visits, lab services, and outside providers.
• Maintain accurate medication storage, ordering, and inventory.
• Support memory care programming with a person-centered approach.

Qualifications

• Current and valid LPN license in the state of Michigan.
• Experience in Assisted Living and/or Memory Care preferred.
• Strong knowledge of geriatric nursing and chronic condition management.
• Excellent communication and interpersonal skills.
• Ability to supervise and collaborate with caregiving staff.
• Detail-oriented with strong documentation and organizational skills.
• Ability to respond calmly and effectively in emergency situations.
