[job_card.job_description]Provide personal care for patients, including bathing, grooming, oral hygiene, and dressing. Support comfort and dignity through activities such as turning, repositioning, back rubs, and fresh linens. Monitor and document patient conditions, reporting changes promptly to the RN Case Manager. Maintain a safe, clean, and comforting environment for each patient. Collaborate with nurses, social workers, and other care team members to ensure continuity of care. Accurately record care provided in accordance with company and regulatory standards. Travel to homes and facilities within your assigned service area to provide compassionate support. Current CNA certification in the state of Illinois. CPR certification (American Red Cross or equivalent). Reliable transportation, valid driver's license, and auto insurance. Prior experience in hospice, palliative care, or home health preferred. Compassionate nature, professional demeanor, and strong communication skills. Dependable, organized, and able to work both independently and as part of a team. A supportive team that celebrates empathy, teamwork, and respect.
