Job Summary :

We're seeking a compassionate and driven Community Support Specialist to join our team, with a focus on supporting adults in various capacities while also assisting individuals with Serious Mental Illness (SMI) and / or Substance Use Disorder (SUD) through our Disease Management program. This role combines the responsibilities of providing mentorship, guidance, and support to adults in the community with the opportunity to make a meaningful impact on the lives of individuals with complex health needs.
