Qualifications and Requirements:

• Valid driver's license and proof of car insurance
• Administers and writes evaluations of assigned patients in accordance with agency policy and procedures
• Must be a licensed Occupational Therapist by the state
• Maintains timely and accurate patient records and reports
• Must have at least one-year clinical experience
• Checks in on Certified Occupational Therapy Assistants after every 4-6 patient visits
• Must be able to travel to prospective patient’s place of residence
