Your Role and Responsibilities

• Performing detailed patient assessments, ordering necessary diagnostic tests, and developing tailored care strategies
• Diagnosing and managing acute and chronic health conditions independently or in partnership with physicians (based on your credentials)
• Providing education on wellness, disease prevention, and disease management to patients and their families
• Prescribing medications and therapies in accordance with your practice scope
• Engaging in comprehensive patient follow-ups and chronic disease management
• Collaborating seamlessly with interdisciplinary teams to ensure integrated patient care
• Maintaining accurate and thorough documentation in electronic medical records (EMR)

Educational and Certification Requirements

• Master of Science in Nursing (MSN) or Doctor of Nursing Practice (DNP)
• Completion of an accredited Nurse Practitioner program

Certification Skills

• Active Nurse Practitioner licensure in the state of New York
• National certification as a Family Nurse Practitioner (FNP-C, AANP, or ANCC) or equivalent specialty
• Basic Life Support (BLS) and Advanced Cardiovascular Life Support (ACLS) certifications (or able to certify within 90 days)
