Internal Medicine or Family Medicine Physician

Position Overview :

Seeking a Primary Care Physician to join a comprehensive care program focused on seniors. This role involves in-person assessments, care coordination with an Interdisciplinary Team (IDT), and oversight of participant care in various settings.

Key Responsibilities :

  • Conduct comprehensive assessments and develop individualized care plans.
  • Collaborate with IDT and community physicians for discharge planning and care delivery.
  • Oversee care in nursing homes, hospitals, and participant homes.
  • Provide direction to Nurse Practitioners and participate in rotating night and weekend call.

Qualifications :

  • Board Certification in Internal Medicine, Family Medicine, or Geriatrics (preferred).
  • Active, unrestricted California Physician License, DEA, and CDS certification.
  • Eligible for Medicare and Medi-Cal enrollment.
