Your Responsibilities

Deliver personalized high-quality care to seniors in their homes assisted living hospice and nursing facilities.

Conduct comprehensive assessments and develop tailored care plans for older adults.

Manage chronic conditions with skill and empathy for patients and families.

Collaborate with cross-functional partnersincluding palliative psych and neuroto deliver coordinated whole-person care.

Build trusted relationships with residents caregivers and facility partners to enhance engagement and referrals.

Maintain consistency in documentation follow-through and patient communication.

What Were Looking For

Active unencumbered Physician Assistant license in the state you will be practicing.

Minimum of 12 years of clinical experience (geriatrics or primary care preferred).

Leadership potential or experience : Ideally youve managed a panel successfully and are ready to grow into leadership roles.

Experience in home-based care SNF or ALF settings strongly preferred.

Compassionate patient-centered approach and strong clinical judgment.

Valid drivers license and reliable transportation.
