Job Title : Case Manager
Division / Program : TAY FSP
Starting Compensation :
22.00 - 28.00 USD Per Hour
Working Location : Fresno, CA
Working Hours / Shift :
Tuesday-Friday 8am -6pm. Working hours are approximate and may include flexibility as needed to meet client needs.

What you bring to SBHG :
- Associate degree in a social service field plus two (2) years of experience working in a mental / behavioral health field required (Internship may be substituted for a portion of the work experience) OR
- Bachelor’s degree in a social science field plus six (6) months experience preferred. (Internship may be substituted for a portion of the work experience)
- Valid California Driver’s License required and must meet Agency driver eligibility criteria.
How you will make a difference :
Provides services at the TAY Center, in the community, and in TAY’s homes. Uses personal vehicle to drive within the community to provide services and to transport TAY and / or TAY’s family members in personal vehicle.Documents all services provided clearly and concisely within the Electronic Medical Record within the timelines and framework established by Stars.Assists the primary therapist with initial and ongoing assessments of the client and family to identify strengths and needs.Monitors the clients’ progress toward goals outlined in their treatment plans. Informs primary therapist of progress.Develops resource directories related to housing and social support resources.Assists clients with securing housing.Division / Program Overview :
