Schedule : M-F, 8-hour shifts

FlexStaff is seeking an experienced Registered Nurse for a Field RN position for our client, a leader in home health care. This position will cover cases in Rockland County, NY.

This position will manage all aspects of patient care related to services provided in the home.

Responsibilities :

• Conduct home care assessments
• Perform home visits
• Complete all required documentation
• Communicate effectively with Primary Care Physicians (PCPs)
• Update medication profiles
• Coordinate care with other providers
• Review and complete case communication notes
• Participate in discharge planning activities
• Other duties as assigned
