Only Angels Home Health and Hospice, LLC in Wichita, KS is looking for a RN to join our team. We are located on 1624 S Rutan Ave. Our ideal candidate is a dedicated, self-starter with both home health and hospice experience.

Responsibilities
Evaluate the effectiveness of nursing service, to the patient and family, on an ongoing basis.Perform admission, transfer, re-certification, resumption of care, and discharge OASIS for the home care patient.Prepare, and present, patient's record to the Clinical Record Review Committee, as indicated.Consult with the attending physician, concerning alterations of Patient Care Plans; checks with the appropriate supervisor and makes changes, as appropriate.Make frequent visits to the patient to evaluate their conditionCoordinate patient servicesSubmit clinical noes, no less often than weekly, and progress notes and other clinical record forms, outlining the services rendered, as indicatedProvide support and comfort according to individual patient’s needsWork with caregivers to plan and execute excellent patient careHospice : Alleviate sudden symptoms of the disease’s end phaseWilling to take On-Call weekendsSubmit a tally of patient care visits made each dayOther duties outlined in the KCHAP job descriptionRequirements and Skills
Proven experience as hospice nurse / registered nurseKnowledge of nursing care processes and medication guidelines / side-effectsCommitted to safety precautions and high ethical standardsSound judgement and attention to detailExcellent communication skills and a compassionate natureCurrent CPR certificateJob Conditions / Physical Requirements
Must have a driver's license and be willing and able to drive to patient's residencesThe ability to access patients' homes which may not be routinely wheelchair accessible is required.Hearing, eyesight and physical dexterity must be sufficient to perform a physical assessment of the patient's condition and to perform and demonstrate patient carePhysical activities will include walking, sitting, stooping and standing and minimal to maximum lifting / turning of patients'Must be able to lift up to 50 lbs.The ability to communicate both verbally and in writing is required as frequent communication by telephone and in writing in English is requiredQualifications
