As the Director of Hospital Case Management, you will oversee and coordinate all departmental activities to ensure compliance with organizational policies and regulatory agencies. Your leadership will drive the integration of patient care and discharge planning processes, optimizing clinical resource utilization and ensuring continuity of care. You will manage daily operations, engage interdisciplinary teams, and achieve established outcomes, all while maintaining an executive presence and excellent communication skills.

Compensation: Mid-Range: $146,000 + 15% Potential Bonus Incentive
Sign-On Bonus: $10,000
Relocation: Case by Case Basis
Schedule: Full-Time, No Weekends
