• Base salary range: $125,000 - $130,000+ (flexible depending on experience)

Responsibilities

• Monday - Friday 8AM - 5PM
• No nights or weekends
• Minimal call responsibility (1 week, 3-4 times per year; triaged through call center)
• Deliver comprehensive, value-based primary care to a geriatric patient population
• Manage small patient panels (12-16 patients/day) in an outpatient setting
• Conduct in-clinic care with occasional home visits and telehealth appointments
• Perform physical exams, assessments, and diagnostic testing
• Create and implement individualized care plans focused on chronic disease management
• Educate patients and families on medication, nutrition, prevention, and disease progression
• Collaborate with a multidisciplinary team including MA, social worker, pharmacist, and specialists
• Document care accurately to support risk adjustment and quality metrics
• Prescribe medications within state scope of practice

Requirements

• Master's degree in Nursing
• Board certification as a Nurse Practitioner (Adult, Adult-Gero, or Family)
• Minimum of 2 years outpatient primary care experience
• Autonomous practice capability required
• Experience with value-based care preferred
• Strong background in geriatrics and chronic disease management
• Proficiency with EMRs (Athena preferred)
• Must hold an active NP license in the state of California and DEA certification (or be eligible)
• Spanish speaking required
• Experience in wound care, ABIs, injections, or imaging interpretation preferred
