The APRN / PA supports the clinical operation of the Transplant Institute with primary focus on clinical services in the inpatient and outpatient setting. Delivery of patient care is conducted in collaboration with the transplant surgeons and physicians including, but not limited to, physical exams, diagnosis and treatment, ordering and interpreting tests, counseling on preventative health, writing prescriptions and assisting in surgery. In addition, the APRN / PA is responsible for coordinating and managing the patient care of high-risk patient populations throughout the continuum to optimize the patient's clinical, functional and psychosocial status, to provide the communication link for coordination of care among the multidisciplinary healthcare team involved with this patient type, and to orchestrate a performance improvement program that focuses on improving patient outcomes. Under administrative guidance and physician supervision, provides comprehensive primary care services, acute care services and extended care services in the area of nursing and / or medicine for individuals and their families. Actively participates in outstanding customer service and accepts responsibility in maintaining relationships that are equally respectful to all.

This includes communicating as necessary with insurer case managerDeveloping and instituting candidates' plan of careObtaining necessary consent for transplant evaluation in accordance to United Network of Organ Sharing (UNOS) guidelinesMaintaining communication with referring physicians, healthcare providers and payersEvaluates and treats patients with acute, chronic complaints and health maintenance concerns which include the following :Assessment of patient health status through physical exam, reviewing patient history, diagnostic tests and procedures and reviewing labs and other critical data.Diagnosis of acute and chronic conditions affecting all organs systemsInitiating orders for medications, laboratory and other diagnostic testing and medical treatments on appropriate formsPrescribing interventions and performing therapeutic measures to ensure the appropriate individualized plan of care is implemented. Applying knowledge of age-specific needs, i.e., pediatric, adolescents, adult and geriatric patients related to plan of care interventions and patients' responses to careEvaluating patient's mental status, substance abuse and referring to mental health treatment as clinically indicatedAssessing patient's compliance and their adherence to medication regimen, treatment recommendations and lifestyle behaviors modificationsAssessing clinical factors including, but not limited to, immunosuppression complications, drug toxicity, infections complications and risk rejectionAssessing patient's functional statusAssessing patient's caregiver and transportation planCoordinating care of recipient and / or living donor as neededCommunicating frequently with referring physicians regarding the patient's statusProviding on-call coverage for medical triage for outpatients in collaboration with the physician on-call.Implementing changes in drug treatment therapy as ordered by the physician(s) and / or healthcare providersCall responsibilities determined by program-specific needsAttends and participates in transplant team meetings including Patient Review Committee, hospital interdisciplinary rounds, discharge planning and QAPI.
