Family Practice - Geriatrics Physician

Schedule: Mon - Fri (No weekends)

Position Overview:

We are seeking a Board-Certified Primary Care Physician (Internal Medicine or Family Medicine) to join a collaborative clinical team in Oakland, CA. This full-time role focuses on delivering comprehensive, patient-centered primary care using evidence-based medicine and national practice guidelines.

The ideal candidate is passionate about preventative care, chronic disease management, and building long-term relationships with patients while promoting coordinated, cost-effective healthcare delivery. New graduates are encouraged to apply.

Key Responsibilities:

• Provide comprehensive primary care services for patients with undiagnosed signs, symptoms, or health concerns

• Focus on health promotion, disease prevention, and health maintenance

• Conduct Annual Health Assessments (AHA)

• Manage Chronic Care Management (CCM) programs

• Provide counseling and patient education

• Deliver direct and indirect patient care as part of individualized treatment plans

• Identify actual or potential health risks based on screening data

• Utilize Electronic Health Records (EHR) to manage workflows, documentation, and clinical compliance

Qualifications:

• MD or DO degree

• Board Certified in Internal Medicine or Family Medicine

• Successfully completed residency in specialty

• Active and unrestricted California Medical License

• Active and valid DEA license

• Active and valid NPI number

• Minimum 1-2 years of clinical experience preferred
