RN Patient Care Manager, Home Health

We are seeking a dedicated and experienced RN / Patient Care Manager in Home Health to join our team. Key Responsibilities :

• Oversee the day-to-day clinical operations of the home health team ensuring compliance with healthcare regulations and standards.
• Coordinate patient care plans working closely with physicians therapists and other healthcare providers to deliver comprehensive care.
• Supervise and support nursing staff providing guidance and assistance as needed to maintain high standards of care.
• Conduct regular evaluations of patient care services implementing improvements to enhance service delivery.
• Facilitate communication between patients families and healthcare providers to ensure clear understanding of care plans and expectations.
• Monitor patient progress and adjust care plans as necessary to achieve optimal health outcomes.
