Inpatient RN Care Manager Responsibilities

Coordinates the care and services of selected inpatient populations to facilitate a safe and timely transition to the next appropriate level of care.

Clinically assesses, plans, implements, coordinates, monitors, and evaluates options and services to meet individual patient health needs.

Partners with the healthcare team to address clinical, psychosocial, and financial patient needs to ensure timely progression of care and safe transition from acute care.

Promotes effective utilization and monitoring of healthcare resources.

Collaborates with the interdisciplinary team to develop and execute comprehensive case management plans to achieve optimal clinical and resource utilization outcomes.

Reviews clinical findings and diagnostic reports and works with physicians to develop comprehensive case management plans of care.

Identifies high-risk patients requiring psychosocial intervention and refers them to Social Services as appropriate.

Conducts patient assessments to proactively prevent readmissions and evaluates readmitted patients to identify causes and implement prevention strategies.

Develops, documents, and implements discharge and transition-of-care plans in collaboration with the interdisciplinary team, modifying plans as patient needs change.

Communicates payer and financial issues that may impact post-acute care planning to patients, caregivers, and the healthcare team.

Monitors length of stay (LOS), actively participates in Care Rounds, and works with the care team to meet LOS and clinical targets while addressing discharge barriers.

Coordinates transitions of care through active communication with patients, families, physicians, care team members, and the Utilization Review Nurse.

Maintains working knowledge of Federal, State, and regulatory agency requirements, including Joint Commission, CMS, Medicare regulations, Observation status, the 2 Midnight Rule, and Medicare Inpatient Only Procedures.

Provides education and resources related to progression of care, utilization of services, appropriate level of care, and safe patient transitions.

Identifies potential risk management or quality issues and escalates through appropriate Chain of Command channels.

Inpatient RN Care Manager Qualifications

Required Skills & Experience :

Excellent communication, negotiation, and conflict resolution skills

Strong data and computer skills

Knowledge of state and federal regulatory standards related to Case Management

Rapid cycle change or clinical performance improvement experience

Prior clinical experience managing hospitalized patients

Preferred Skills & Experience :

Acute care case management experience

Case management training from a professional Case Management organization

Inpatient case management experience (2 years preferred)

Case Manager Certification preferred

Previous travel experience preferred
