This organization offers a highly coordinated, patient-centered model that integrates medical, social, and behavioral services for individuals who require more support than the typical primary care environment provides. As the Clinical Team Lead, you ll guide a small care team while managing a manageable panel of patients through relationship-driven, longitudinal care.

Key Responsibilities

• Lead and support a cross-functional care team delivering comprehensive primary care

• Provide both clinic-based and occasional in-home visits to enrolled patients

• Emphasize continuity and prevention rather than episodic or volume-driven care

• Collaborate closely with behavioral health, social work, therapy, and nursing professionals

• Participate in a light call rotation (phone-only, a few nights or weekends per month)

Compensation and Benefits

• Base salary: $315,000 to $380,000, depending on experience

• Quarterly performance incentives

• Sign-on bonus: $10,000

• Relocation assistance: Up to $50,000

• Board Certified or Board Eligible in Internal Medicine, Family Medicine, Geriatrics, or Emergency Medicine

• Eligible for Medicare and Medicaid provider enrollment in California
