WSi Healthcare is seeking a compassionate and experienced Registered Nurse (RN) Case Manager for a full time home health position in the Colorado Springs area. Responsibilities

• Manage a diverse caseload and oversee individualized patient care plans.

• Coordinate care with physicians, therapists, and support teams to ensure comprehensive patient outcomes.

• Supervise and guide Home Health Aides (HHAs) to maintain clinical quality standards.

• Conduct patient assessments, provide education, and support patients and families through the care process.

• Maintain accurate and timely documentation in compliance with regulatory and agency standards.

• Participate in an on call rotation (approximately one weekend per month).
