Location - Ohio

1099 locums contract

Looking for several APP's to provide telehealth visits for post-partum and well-child visits on ages 3 to 21 providers will perform well adults visits also. Providers will not be responsible for acute care, chronic illness management, or prescribing of medications (providers will be the first link into the healthcare system and refer out for ongoing treatments

Duties:

Post-partum visits – Perform a post-partum assessment including the Edinburgh depression screening and refer for any needs discovered.

Well-child visits – Assess achievement of age-appropriate developmental milestones and provide anticipatory guidance. Assess immunization status and refer to close gaps.

Outbound calls – Reach out to patients who no-show appointments. Perform outbound calls to patients in any down time (ie no-shows or schedules not being filled).

Collaborate with assigned physician.

Qualifications:

Ohio license family nurse practitioner or physician assistant.

18 months of related work experience.

Ability to enroll as a provider in state Medicaid program.

Fast & reliable Wi-Fi (Starlink not compatible)

HIPAA compliant workspace – Dedicated workspace behind closed door

Schedule:

Min number of hours per week: 20 - prefer fulltime providers but will look at all

Afternoon & Evenings (12p – 10pm) are highest pt. volume

Weekend coverage is highly preferred

Open 7 days a week, 8am to 10pm

Other Information:

DEA is not required

Remote Work Package will be sent– (2 monitors, laptop, headset, keyboard & mouse)

Training:

Three days of paid training, hands on and autonomous work, on-going support

Microsoft Teams, Elation (EMR), Genesys (outbound call system)
