Job Title:* Physical/Occupational Therapist, PRN

Job Summary:

We are seeking a compassionate and experienced Physical Therapist to join our Home Health Care team. As a Home Health Care Physical Therapist, you will provide high-quality, patient-centered care to patients in their homes, focusing on improving mobility, strength, and function.

Responsibilities:

• Conduct initial and ongoing assessments of patients' physical therapy needs

• Develop and implement individualized treatment plans

• Provide skilled physical therapy interventions, including exercises, manual therapy, and modalities

• Educate patients and caregivers on proper techniques and strategies

• Monitor patient progress and adjust treatment plans as needed

• Collaborate with interdisciplinary teams, including physicians, nurses, and occupational therapists

• Document patient information accurately and timely

• Participate in case conferences and quality improvement initiatives

Requirements:

• Bachelor's or Master's degree in Physical Therapy

• Current PT licensure in the state of Florida

• Minimum 1-2 years of experience in physical therapy, preferably in home health care

• Strong clinical skills, critical thinking, and problem-solving abilities

• Excellent communication, interpersonal, and patient education skills

• Ability to work independently and as part of a team

• Reliable transportation and ability to travel to patient homes

Preferred Qualifications:

• Home health care experience or certification (e.g., HHC PT, OASIS)

• Knowledge of Medicare regulations and guidelines

• Experience with electronic medical records (EMRs, Wellsky/Kinnser) and mobile documentation systems
