In this full-time role, you will work with a diverse range of older adults, providing thorough, thoughtful, and comprehensive Risk Assessments in both clinic and home health environments. Your assessments will help guide long-term care planning, reduce avoidable hospitalizations, and support patients in maintaining independence and quality of life.

Job specification

• Work with patients to complete comprehensive Risk Assessments in both clinic and home health settings.

• Provide high-quality patient evaluations with a focus on geriatric health and preventive care.

• See an average of 8 patients per day in the clinic setting.

• Complete 4-5 home health patient visits per day when assigned.

• Maintain accurate, timely, and compliant clinical documentation.

• Collaborate with interdisciplinary healthcare teams to support patient care plans and follow-up needs.

• Identify health risks, evaluate chronic conditions, and provide patient education to support long-term wellness.

• Opportunity to pick up extra shifts to increase earnings if desired.

Candidate requirements

• Board Certified Nurse Practitioner - Required

• Experience working with geriatric patients - Required

• Bilingual in Spanish, Mandarin, or Cantonese is a plus, but not required

• Strong communication, assessment, and interpersonal skills

• Ability to work independently and manage clinical responsibilities with minimal supervision
