Java Backend Developer

We are seeking a compassionate and dedicated Social Worker to support individuals, families, and communities by assessing needs, connecting clients to essential resources, and providing ongoing case management. The ideal candidate brings strong communication skills, empathy, and the ability to navigate complex social, emotional, and behavioral challenges.

Responsibilities

• Conduct client assessments to evaluate social, emotional, mental health, and support needs.
• Develop individualized care plans and coordinate services to meet client goals.
• Provide crisis intervention, counseling, and support to clients facing difficult life situations.
• Connect clients to community resources such as housing, healthcare, financial aid, and behavioral health services.
• Maintain accurate and timely case notes, reports, and documentation in accordance with state and federal guidelines.
• Collaborate with healthcare providers, schools, law enforcement, and community agencies to ensure holistic support.
• Advocate for clients and help them navigate systems such as Medicaid, social services, and educational support programs.
• Conduct home visits, community outreach, and follow-ups to track client progress.
• Participate in multidisciplinary team meetings and case reviews.
• Ensure compliance with HIPAA, confidentiality laws, and organizational policies.
