RN Case Manager - Home Care in Minneosta

Key Responsibilities

• Conduct comprehensive head-to-toe assessments, care coordination, disease / medication management, skilled wound care, catheter / ostomy care, infection prevention, and home safety evaluations.
• Visit patients in their homes to evaluate care quality, review clinical notes, and participate in ongoing interdisciplinary assessments.
• Implement physician-ordered services, educate patients / families on healthcare needs, and complete timely, accurate clinical documentation.
• Collaborate with the healthcare team to ensure seamless coordination, progress tracking, and discharge planning.
