Rehab Tech / Case Manager – 2nd & 3rd Shift (Weekends Required)

Position Overview :
We are seeking a compassionate and dependable Rehab Tech / Case Manager to support patients in their daily functioning, recovery goals, and coordination of care. This role plays a key part in providing direct support, fostering therapeutic relationships, and ensuring continuity of services across clinical and community settings.
Key Responsibilities :
- Build and maintain supportive, therapeutic relationships with individuals receiving services.
- Provide safe, effective, and efficient direct care in accordance with established policies, procedures, and standards of practice.
- Make appropriate recommendations regarding living environments, therapy needs, psychiatric follow-ups, and case management resources.
- Deliver services outlined in each individual’s Service or Recovery Plan.
- Accurately document daily interactions and communicate relevant updates to the clinical team.
- Maintain knowledge of local community resources and assist individuals in accessing them appropriately.
- Assist individuals with Activities of Daily Living (ADLs), including personal care and daily task support.
- Support individuals in obtaining eligible benefits and completing required documentation.
Schedule :
2nd and 3rd ShiftsWeekend availability required
