Carter Hospice provides a team-oriented approach which includes expert medical care and pain management, along with emotional and spiritual support. Hospice care is tailored to the patient’s needs and wishes, while support is also provided to the patient’s loved ones and family.
The Nurse Practitioner works under the supervision of the Clinical Coordinator to provide, facilitate, and assist with care of the hospice agency’s patients. Under the supervision of a physician, the Nurse Practitioner visits patients, completes physical assessments, performs face-to-face assessments, and evaluates hospice eligibility as assigned in consultation with the hospice interdisciplinary team. The Nurse Practitioner builds from the resources of the community to plan and direct services to meet the needs of individuals and their families.

Qualifications:
- Graduate of an accredited School of Nursing.
- Graduate of an accredited Nurse Practitioner Program.
- Current Advanced Practice Registered Nurse (APRN) Licensure.
- Minimum of 2 years’ experience in the Nurse Practitioner role preferred.
- Is familiar with the philosophical and technical aspects of hospice care.
- Is familiar with principles of quality managements and ethical issues in hospice care.
- Experience in pain and symptom management.
- Experience in hospice preferred.
Responsibilities:
- Perform face-to-face encounters with the patient to determine eligibility for hospice services and evaluates patients to provide information for recertification discussions.
- Collaborates with Hospice Medical Directors, Physicians, and the Agency’s Multidisciplinary team regarding clinical issues and patient/family needs.
- Participates in interdisciplinary group discussions and meetings when appropriate or requested.
- Communicates and collaborates with the interdisciplinary group to create, review and revise the patient's plan of care.
- Reviews patient charts and medications as requested and provides input on appropriate interventions or changes in the plan of care.
- Understands regulations and standards particular to state and federal hospice requirements.
- Reviews written policies and procedures.
- Makes home visits to the patients and families as needed.
