This form needs to be 100% accurate and have all request noted in the highlights section.
Start Date should be on or very quickly after the Order Start Date listed.
All RTO should be listed on the cover page, must be less than 7 days.
RTO will not be approved in the first week of assignment or holidays.
All facilities the traveler is open to working in should be listed on the submission.

24 months minimum of current MRI experience - REQUIRED
Certifications must be American Heart Association OR client, active upon submission and INCLUDED IN SUBMISSION - REQUIRED
No License Hits - REQUIRED
Banner will not approve RTO within the first week or holidays. Can not exceed 7 days.
