Job Summary

We are seeking a dedicated Principal Care Management (PCM) Coordinator to join our clinical care team. This role supports patients with chronic and complex conditions by coordinating care, facilitating communication between providers, and ensuring that each patient receives the highest standard of personalized support.

The ideal candidate is empathetic, detail-oriented, and comfortable managing multiple priorities while maintaining strong relationships with both patients and healthcare professionals.

Key Responsibilities

• Serve as a key advocate for patient care by identifying and supporting those eligible for Principal Care Management (PCM) services.
• Implements patients individualized plan of care who have a single chronic condition or with multiple chronic conditions but focused on a single high-risk condition.
• Coordinate individualized care plans for patients with chronic conditions, ensuring consistent follow-up and proactive management.
• Communicate regularly with patients to monitor progress, provide education, and adjust care plans as needed.
• Support providers in developing and implementing care management strategies.
• Track all PCM services and document interactions accurately within the EMR.
• Triage patient phone calls and messages, offering medical guidance per established protocols.
• Educate patients on autoimmune and chronic disease management.
• Encourage adherence to treatment plans, follow-up visits, and lifestyle recommendations.
• Collaborate with physicians, clinical staff, and external partners to ensure comprehensive, coordinated care.
• Participate in data tracking and outcomes reporting to enhance program quality and effectiveness.

Pay Range : $22.00 – $25.00 per hour (based on experience)
