• Monday through Friday schedule with 32 guaranteed hours weekly
• 30 units weekly productivity requirement
• Home health setting with service area coverage
• On-call availability required if requested
• HCHB and Oasis experience required
• 2 years physical therapy assistant experience required
• Geriatric population care focus
• OASIS assessment experience required
