Your Role as a Hospice RN / RN Case Manager :

Act as the primary contact for patients and families, guiding them throughout their hospice journey.

Administer and document skilled, personalized nursing care based on each patient's unique Plan of Care in their home, assisted living, or wherever they reside.

Conduct initial and ongoing assessments to monitor patient conditions and modify care as necessary.

Collaborate with a multidisciplinary team including physicians, LPNs, CNAs, social workers, and chaplains to tailor care plans.

Provide emotional, physical, and spiritual support to patients and their loved ones, including education and bereavement guidance.

Train caregivers on safe and confident care practices, providing practical support and emotional reassurance.

Communicate significant updates about patient conditions to the healthcare team to ensure timely interventions.

Maintain accurate clinical documentation and adhere to infection control and compliance standards.

Support the organization's quality improvement initiatives to maintain compassionate and efficient care.

Assist patients and families with understanding key topics including :

• Medication management
• Hospice philosophy and services
• Symptoms and pain management
• End-of-life processes and expectations

What You Bring :

Active RN license in the state of employment (or eligibility to obtain one).

Minimum of 1 year of RN experience in hospice, home health, ICU, oncology, geriatrics, or related nursing fields.

A genuine compassionate nature centered on patient care.

Ability to provide care in diverse settings, including private homes and medical facilities.

Strong communication and critical thinking skills, especially in sensitive situations.

Valid driver's license, auto insurance, and reliable transportation.

Current CPR certification required.

Preferred Experience (Not Required) :

Hospital-based RN experience (Med-Surg, ICU, ER, PACU, telemetry, cardiac).

Experience as an Admissions RN, wound care nurse, travel nurse, or float pool assignments.

Experience working with terminally ill patients or serious illness support.
