The opportunity will allow you to build your own patient panel. Patient panels are set to a maximum of 500 members.

Position Description :

• Care for members includes chronic disease management and acute outpatient care as well as comprehensive annual preventive health physical examinations
• No in-person night or weekend responsibilities
• No inpatient rounding responsibilities but can collaborate and advocate for members when hospitalized
• Provide 24 / 7 access to members for after-hours urgent needs
• Opportunities to craft your scope of practice to your unique skillset (lifestyle medicine, procedures, etc.)
