Job Summary
We are seeking a Community Outreach Liaison to join our team! As a Community Outreach Liaison, you will spend your time reaching out to medical offices, doctors, hospitals, Board and Cares, Assisted Living Centers and organizations to talk about the Rehabilitation Services at our Adult Day Health Care Center, take time to understand what the potential participants and referring sources are looking for to meet rehabilitation service needs. You will also manage relationships with existing clients and be knowledgeable on the services offered and to be a true resource for our clients. The ideal candidate has demonstrable community outreach experience, exceptional communication and interpersonal skills. On the job training will be provided to learn about our Center Rehabilitation goals and services.

Responsibilities

• Reach out to new and potential clients to form relationships, assess needs, and provide information on the Center's rehabilitation program.
• Maintain existing relationships with clients to present the Center's rehabilitation program and assist with issues
• Maintain excellent knowledge of the Center's various rehabilitation treatment goals and services.

Qualifications

• Demonstratable community outreach experience desired
• Strong communication and interpersonal skills
• Strongly self-motivated, and very goal oriented
