Job Roles

• Provide care in a manner which ensures participant satisfaction and retention, and is in compliance with all regulatory requirements and guidelines.
• Assist with instrumental Activities of Daily Living (IADLs) across care settings.
• Assist participant in their home with light housekeeping, medication reminder, and meal preparation.
• Provide personal care to include bathing, dressing, toileting, oral and denture care, hair and skin, shaving, nail trimming, etc.
• Help participant's to move, turn, get in and out of bed, chair or wheelchair, and ambulate with or without assistive devices.
• Provide physical assistance and attendant care across care settings.
• Identify safety hazards and emergency situations. Initiate corrective action as appropriate and report situation to charge person / supervisor.
• Measure and record vital signs / weights and reports information to appropriate medical staff following established procedures and protocols.
• Observe and report to immediate supervisor any changes in participant's physical or mental status; record such observations on appropriate records.
• Participate in case conferences with other members of the interdisciplinary care team as appropriate.
• Help maintain order and cleanliness of the multi-purpose PACE Center. Assist with dining area set-up and distribution of meals and snacks as assigned.
• Assist with multi-purpose PACE Center activities as assigned.
• Perform other duties as assigned.

In the spirit of pay transparency, we are excited to share the base range for this position, exclusive of fringe benefits. $21.50 - $25.50

If you are hired at Family Health Centers of San Diego, your final base salary compensation will be determined based on factors such as geographic location, jurisdictional requirements, skills, education, and / or experience. In addition to these factors, we believe in the importance of pay equity and consider internal equity of our current team members as a part of any final offer. Please keep in mind that the range mentioned above is what we reasonably expect to pay for the role. Hiring at the maximum of the range would not be typical in order to allow for future and continued salary growth. We also offer a generous compensation and benefits package.
