RN / Patient Care Manager, Home Health

We are seeking a dedicated and experienced RN / Patient Care Manager in Home Health to join our team. Key Responsibilities:

• Oversee the day-to-day clinical operations of the home health team, ensuring compliance with healthcare regulations and standards.
• Coordinate patient care plans, working closely with physicians, therapists, and other healthcare providers to deliver comprehensive care.
• Supervise and support nursing staff, providing guidance and assistance as needed to maintain high standards of care.
• Conduct regular evaluations of patient care services, implementing improvements to enhance service delivery.
• Facilitate communication between patients, families, and healthcare providers to ensure clear understanding of care plans and expectations.
• Monitor patient progress and adjust care plans as necessary to achieve optimal health outcomes.

Qualifications:

• Registered Nurse (RN) with a valid nursing license in the state of operation.
• Minimum of 3 years of nursing experience, preferably in a home health setting.
• Prior experience in a managerial or supervisory role is highly desirable.
• Strong organizational and communication skills.
• Ability to work independently and manage multiple priorities effectively.
