A company is looking for a Wellness Coordinator.
Key Responsibilities :
Provide care coordination and administrative support for members with behavioral health and substance abuse needs
Engage members, screen for care gaps, and develop individual care plans
Document in electronic systems and coordinate information with clinical team partners
Required Qualifications :
Bachelor's degree in Human Services, Social Work, Psychology, Early Childhood Development, or related field
2+ years of managed care or call center experience
1+ years of computer proficiency experience
1+ years of experience handling sensitive issues with members and providers confidentially
Access to a designated quiet workspace at home with secure internet for handling Protected Health Information (PHI)
