Your Role as the MDS Coordinator (RN)

Americare Senior Living is seeking a detail-oriented and clinically skilled MDS Coordinator (RN) to support accurate assessments, regulatory compliance, and quality resident outcomes within our skilled nursing community.

In this role, you'll coordinate the Minimum Data Set (MDS) process in accordance with federal and state guidelines while supporting resident care, interdisciplinary collaboration, and a safe, home-like environment. Your expertise will play a key role in care planning, reimbursement accuracy, survey readiness, and overall quality of life for our residents.

Summary

The MDS Coordinator (RN) is responsible for coordinating and completing the MDS process as required by federal and state regulations.

This role ensures accurate, timely, and compliant clinical documentation, supports care planning and Medicare processes, and collaborates with nursing leadership, the Administrator, and the interdisciplinary team. The MDS Coordinator assists in maintaining facility standards, supporting inspections, and promoting quality outcomes for residents.

This role may also be known as:

MDS Coordinator (RN), Clinical Reimbursement Nurse, MDS Nurse, Assessment Coordinator, or RN Case Mix Coordinator.

What You'll Do

• Coordinate and complete MDS assessments in accordance with federal and state guidelines
• Ensure admission assessments, nursing notes, care plans, and required clinical documentation are accurate and current
• Complete assigned MDS sections, assessments, and RAP summaries, including but not limited to:
• Skin integrity, contractures, falls, incontinence, catheters, side rails
• Feeding tubes, dehydration, oral and dental care, pressure ulcers
• Psychotropic medications and physical restraints
• Review, update, and maintain resident care plans based on assessment findings
• Participate in Medicare meetings, care plan meetings, and interdisciplinary team discussions
• Assist the Director of Nursing with maintaining compliance with state, federal, and Americare standards
• Support survey readiness and participate in inspections as needed
• Ensure medication administration systems meet resident needs and regulatory guidelines
• Communicate significant changes in resident condition to the Administrator and nursing leadership, including hospital transfers or other critical events
• Assist in determining resident admissions, placements, and room transfers using established nursing criteria
• Schedule, participate in, and maintain records for nursing staff meetings as needed
• Review incident reports routinely and ensure appropriate documentation and follow-up
• Assess nursing supply and equipment needs and ensure adequate inventory levels
• Provide on-call support to staff as needed for matters related to nursing care
• Perform other duties as assigned by the Administrator

What You Bring

Education

• Graduate of an accredited school of nursing

Licensure / Certification

• Current Registered Nurse (RN) license required
• Meets all state and federal licensing and continuing education requirements

Experience & Skills

• Experience with MDS 3.0 and skilled nursing required
• One (1) or more years of clinical experience in a Skilled Nursing Facility preferred
• Two (2) or more years of long-term care or assisted living experience preferred
• Knowledge of Medicare reimbursement, RUGs, and state and federal regulations
• Strong attention to detail, organization, and analytical skills
• Ability to collaborate effectively with interdisciplinary teams
• Clear and professional communication skills

Physical Demands

• Stand, sit, walk, bend, reach, and change positions as needed throughout the workday
• Lift up to 25 lbs. as required
• Push, pull, and maneuver up to 100 lbs., including resident mobility assistance
• Perform tasks requiring visual acuity, hearing, and verbal communication
• Use electronic health record systems and standard office equipment regularly
