Housing Case Manager & Social Services Coordinator Duties and Responsibilities
-Assists clients in achieving their housing goals
-Willingness to work with clients with serious mental illness
-Develops a person-centered service plan for each client
-Assesses each client's needs, with an emphasis on strengths and barriers related to getting & keeping their housing
-Works with property management staff to help clients maintain their housing
-Identifies strategies that will help clients maintain their housing and strengthen their recovery
-Coordinates/facilitates speakers, groups or events that foster community, identify helpful resources, and reinforce wellbeing
- Supports clients in understanding the basic expectations of being a tenant, lease requirements, household maintenance, etc.
-Collaborates with landlords and other community partners, advocating for client needs and rights.
-Assists clients in applying for rental assistance, SNAP benefits, social security income, etc.
-Participates in regular supervision and team meetings
