Grab Healthcare is seeking a compassionate and self-motivated

Physician Assistant (PA)

to join our growing team. In this field-based position, you?ll provide high-quality primary care services in assigned

skilled nursing facilities (SNFs), assisted living facilities (ALFs), and private homes

in

Salt Lake County .

You?ll have the opportunity to make a meaningful impact by delivering patient-centered care to vulnerable populations, while enjoying the independence and flexibility of working in diverse care settings.

What You?ll Do

Conduct in-person medical visits at assigned facilities and home settings

Perform routine and acute evaluations, manage chronic conditions, provide preventive screenings, and oversee medication management

Collaborate closely with facility staff, mobile medical assistants (MAs), and telehealth support teams to ensure coordinated care

Order and interpret labs and diagnostics, creating and following through on comprehensive care plans

Document clinical encounters accurately and efficiently in the EHR system

Communicate with patients, families, caregivers, and interdisciplinary care teams

Follow evidence-based practices, organizational protocols, and state / federal regulations

Participate in team meetings, case reviews, and ongoing quality improvement efforts
