Job Summary and Qualifications
The Registered Nurse Care Coordinator is dedicated to promoting patient-centered care by managing and coordinating the plan of care throughout the patient's stay. You will ensure efficient length of stay management, resource utilization, and develop safe discharge plans in collaboration with a multidisciplinary team. You will facilitate smooth care transitions and promote quality outcomes while optimizing resource use.
