Social Work / Admissions Director

Responsibilities:

• Assists residents with emotional adjustment to facility.
• Assist with admissions
• As directed, interviews new residents and records social history and assessments.
• Completes the social services portion of the MDS.
• Schedules and Attends and participates in interdisciplinary care plan meetings.
• Writes summaries and progress notes as directed.
• Assists in conducting a pre-discharge evaluation
• Assists with discharge planning. Follows up after discharge.
• Writes discharge summaries as assigned.
• Develops a relationship with each resident

Qualifications:

• Associate's Degree in Social Work
• One to Three years of professional experience and/or training; or equivalent combination of education and experience.
