Registered Nurse (RN) Home Health (Days)

Philadelphia, PA

Pay : $3000 / 40 hrs | $32 / hr + $1720 / non tax |13 Weeks | Mileage Reimbursement

8 : 00 AM 4 : 30 PM | 40 hours per week guaranteed

Position Overview :

Redeemer Health Home Care and Hospice is seeking an experienced Home Health RN to provide safe, compassionate, and holistic care to patients in their homes throughout Philadelphia County. The RN will perform skilled nursing visits, support chronic disease management, and educate patients and families to promote independence and positive outcomes.

Job Details

Start Date : 11 / 24 / 2025
Shift : Days, 8-hour shifts
Weekend Requirement : Possible weekends
On-Call : None
Productivity : Approximately 6 visits per day (30 per week)
Mileage reimbursed at $0.62 per mile
Territory : Philadelphia County (ZIPs : 19118, 19119, 19150, 19126, 19138, 19120, 19140, 19141, 19144, 19129, 19128)

Responsibilities

Complete SOC / ROC, revisits, recerts, and discharges

Assess patient conditions and develop care plans aligned with physician directives

Provide education on self-care and disease management

Coordinate services with interdisciplinary team members

Maintain accurate and timely documentation via Homecare Home Base (HCHB)

Ensure compassionate, patient-centered care in the home environment

Requirements

1+ year of recent Home Health experience (required)

Case management experience (required)

Proficiency with Homecare Home Base EMR (required)

Valid Pennsylvania RN license (must be active at time of submission)

BLS (AHA) certification (active)

Clear driving record, valid drivers license, and current auto insurance

Preferred Qualifications

Previous travel nursing experience

IV and venipuncture experience

Compensation and Benefits

Guaranteed 40 hours per week

Mileage reimbursement : $0.62 per mile
