Ultimate Home Health Care is seeking experienced PRN RN Case Managers to provide high-quality home healthcare to clients primarily in Dallas County, with opportunities in surrounding Dallas–Fort Worth areas as needed. Role Responsibilities:

• Provide professional nursing care in clients’ homes

• Perform patient assessments and manage care plans

• Accurately document care using OASIS/Kinnser

• Manage diverse patient needs in a home health setting

• Respond to PRN and urgent visit needs as assigned

Qualifications:

• Graduate of an accredited nursing program

• Active Texas RN License

• Valid Driver’s License and reliable transportation

• Minimum 1 year of home health experience

• Experience with OASIS/Kinnser preferred

• Ability to travel locally within Dallas County

• Flexible availability for PRN visits

Work Location:

• Client homes throughout Dallas County

• On the road / field-based role
