About the Role
Vanna Health is seeking an experienced, recovery-focused Registered Nurse (RN) who is passionate about community care, health education and facilitating care that addresses all the dimensions of wellbeing. In this role, you will bring passion for serving individuals with mental illness and chronic physical health conditions in Marion, Lake, Allen and Vanderburgh counties, helping them access community-based care to improve health outcomes and quality of care. This role will be fully in-person in our office located in Indianapolis. While you may be expected to provide clinical care to individual members intermittently, we aim to promote continuity of care by supporting members in receiving direct care from their longitudinal outpatient providers, as much as possible. As such, your role will be primarily focused on Vanna Care Team education, care coordination support and population level program development.
Responsibilities
Vanna Care Team Education and Clinical Consultative Support
- Conduct interdisciplinary care team meetings to review members' condition, align on appropriate service coordination as part of the member-centered care plan and promote team collaboration
- Educate Vanna care team members to ensure they can accurately and safely monitor for acute or worsening psychiatric symptoms, medication side effects, and physical health needs and can escalate appropriately
- Provide backup for Vanna RNs at other sites as needed
Care Coordination
Promote care coordination and communication across healthcare systemsAdvise Vanna care team on appropriate services required by members and coordinate services with our clinical partner providers, social service organizations and payersFacilitate Organizational Growth and Program Development
Participate in development of effective population health care management and intervention strategiesDevelop behavior change and education programming for members to maximize healthy behaviors including medication adherenceParticipate in the development and maintenance of partnerships with local community clinical and social services organizationsProvide feedback to Vanna Technology and Operations Teams on existing processes and suggest improvements over timeParticipate in regular Vanna and Payer conferencing on clinical workflow to provide actionable feedback and collaborate directly on identified members of interestDirect Clinical Care As Needed
Conduct nursing physical health assessments for members and assist with acute and chronic medical and mental health conditionsDeliver clinical care in the community, including member homes, inclusive of administering long-acting antipsychotic medications and monitoring of vital signsAdhere to documentation protocols related to medication reconciliation, member appointments, treatment plans, coordination with external providers, and other protocols related to responsibilitiesAbout You
Need to Haves
Minimum of 3 years working with adults with mental illness and substance use disorders in community based settings (inpatient, outpatient, or home health)Minimum of 1 year of experience working as a medical nurse or working as a nurse in inpatient or outpatient medical settingsExperience working in and around the communities of Marion, Lake, Allen and Vanderburgh countiesBelief that individuals suffering from mental illness can and will recover to achieve life goals when provided the opportunity, support, and communityAbility to work in a teamWillingness to learn how to work with Google, Slack, and Vanna EHR toolsBasic computer and phone skillsA valid driver's licenseAbility to commute into our office in Indianapolis daily - this is a fully in-person roleEducation and License Requirements
Associate's Degree in Nursing is requiredUnencumbered registered nurse (RN) license in the state of IndianaNice to Haves
