6 patients per day / Productivity of 30 per week
Must complete revisits SOC / ROCs recerts and discharges
Mileage reimbursed at $0.62 cents per mile

A Home Care Visiting RN provides safe holistic care to a population with multiple co-morbidities and advanced chronic illnesses. The RN educates regarding self-management and provides support to meet the needs of the patient family and community.
Requirement description :
- 1 years of recent Home Health experience - REQUIRED
- Case management experience - REQUIRED
- Homecare Home Base EMR - REQUIRED
- Previous travel experience - Highly Preferred
- IV and venipuncture experience - Highly Preferred
