Per Diem Home Health Physical Therapists needed throughout Maricopa County. West Valley, East Valley, Central Phoenix, South Phoenix and North Phoenix.

Meridian Health Care has been providing "Excellence in Care" to its patients throughout Maricopa County for over 17 years. We offer a flexible schedule, that you make yourself enabling a great work/life balance. Because this is a per diem position you choose the amount of patients and visits.

Responsibilities:

• Develop and implement physical therapy programs that focus on rehabilitation
• Work directly with patients to achieve maximum physical recovery
• Evaluate effects of therapy treatment and communicate patients' progress
• Record and document patient care services
• Collaborate with other team personnel to achieve well-rounded care

Qualifications:

• Previous experience in physical therapy, rehabilitation, or other related field
• Ability to handle physical workload
• Ability to build rapport with patients
• Excellent written and verbal communication skills
• Strong leadership qualities
