Position Summary

We are seeking a dedicated and compassionate Physical Therapist to join our Home Health Care team. In this role, you will deliver one-on-one therapeutic services to older adults in their homes, helping them regain mobility, improve function, and maintain independence. This position offers autonomy, flexibility, and the opportunity to make a profound difference in the lives of seniors and their families.

As a nonprofit organization, Mozaic Senior Life is a qualifying employer for the Public Service Loan Forgiveness (PSLF) Program .

Responsibilities

• Conduct comprehensive evaluations to determine patients' functional needs and develop individualized treatment plans.
• Provide evidence-based physical therapy interventions in the home setting.
• Educate patients and caregivers on exercises, safety techniques, and equipment use.
• Collaborate with an interdisciplinary team including nurses, OT, SLP, social workers, and physicians.
• Document assessments, progress notes, and care plans in accordance with regulatory and organizational standards.
• Adjust treatment plans as needed to ensure optimal outcomes and patient satisfaction.
• Participate in case conferences, team meetings, and quality improvement initiatives.
• Ensure compliance with all home health care regulations and agency policies.
