Position Summary

This is a remote role open to qualified candidates residing in AZ, HI, TX, IL, FL, or PA and holding active, unrestricted pharmacist licensure within their state of residence.

Medication Therapy Management supports the company vision to help people on their path to better health by performing MTM for members at the highest-risk of having medication related problems. The team of pharmacists, technicians, and coordinators work together to identify problems and coordinate care to facilitate resolution to ensure each member is on an optimized medication regimen to help them reach their health-related goals.

The role of the MTM Pharmacist is to deliver telephonic medication therapy management (MTM). Effective delivery of this CVS Health telephonic based MTM service directly results in improved member care through the identification of medication related problems followed by the coordination of the resolution to these problems. This is accomplished by telephonically performing medication reconciliations with high-risk members and communicating (both verbally and written) medication related problems to members, caregivers, and prescribers. The MTM Pharmacist interacts successfully with patients, prescribers, nurse case managers, clients, and internal resources to identify member-specific treatment plans based on nationally recognized treatment guidelines to help the member attain their health related goals and help them on their path to better health. Pharmacists will also be trained to use the innovative skill of behavioral modification and interviewing, which are key drivers to positively impact patient health outcomes.

What you will do

Perform telephonic comprehensive medication reviews, which includes but is not limited to ensuring a complete medication reconciliation is completed while performing clinical assessments on applicable conditions for each targeted member.

Evaluate the medical history, current health status, and medication-related needs of patients to develop and provide personalized medication therapy plans.

Identify, document, and facilitate resolution of identified medication related problems.

Create a written summary (patient takeaway) of each member interaction written at an appropriate level of health literacy.

Create faxed materials intended for prescriber offices to facilitate resolution to medication related problems.

Build and maintain positive relationships with internal and external customers including physicians and nurses.

Other responsibilities as directed by supervisor and / or manger.

Required Qualifications

Must be located in either AZ, HI, TX, IL, FL, or PA and have an active pharmacist license in that state.

1+ years in Retail, LTC, Hospital or PBM pharmacy experience, or other pharmacist-to-patient consultation experience

Experience working with patients with chronic disease states, especially diabetes.

Experience working as a pharmacist in a call center or other remote environment, with proven ability to meet goals in a remote environment

Adequate home internet required. High Speed Internet preferred. Required minimum speeds of 50 mb / sec download and 5 mb / sec upload. INCLUDE SCREENSHOT AT TOP OF RESUME PLEASE.

Previous pharmacy experience delivering MTM, specifically using Outcomes MTM Connect platform

Preferred Qualifications

Pharmacy knowledge, drug utilization review, excellent consultation skills with patients, strong customer service skills.

PC literate, solid writing skills, knowledge of pharmacy laws, and accurate clinical information.

Excellent communication skills.

Ability to learn new operating systems and new communication strategies.

Must be flexible, organized and have a passion for working directly with patients

Pay Range

The typical pay range for this role is :

$45.43 - $101.54

This pay range represents the base hourly rate or base annual full-time salary for all positions in the job grade within which this position falls. The actual base salary offer will depend on a variety of factors including experience, education, geography and other relevant factors. This position is eligible for a CVS Health bonus, commission or short-term incentive program in addition to the base pay range listed above.
