Licensed Practical Nurse (LPN) Outpatient Clinic (Float)

Locations : Ithaca & Cortland NY

Schedule : MondayFriday Day Shift 8-hour shifts Some Saturdays (7 : 30am1 : 00pm)

Shift : Day 5 shifts / week 40 hours / week

Job Summary

The Licensed Practical Nurse (LPN) supports daily clinical operations within outpatient rehabilitation and wellness centers. This role provides medication administration patient assessments and general clinical support while ensuring safe compassionate care. The LPN will float between the Ithaca and Cortland locations (weekly assignments provided in advance) and collaborate closely with RNs and Nurse Practitioners to support patient care needs.

Key Responsibilities

Prepare medication dosing calibration and clinic setup each morning (starting at 7 : 00am).

Administer liquid methadone and occasional injectable medications following strict safety and documentation protocols.

Perform general wellness checks including vital signs blood pressure and patient assessments.

Assist Nurse Practitioners with rooming patients collecting urine samples verifying patient information and preparing exam rooms.

Provide compassionate patient-centered care to individuals with unique or complex medical and psychosocial needs.

Maintain strict attention to detail when handling controlled substances; medication errors cannot be made.

Document all clinical activities using Epic for charting and Evolve for dosing.

Participate in Saturday rotation (7 : 30am1 : 00pm).

Collaborate with on-site RN and NP during all shifts.

Uphold all safety compliance and regulatory standards.

Perform additional duties as assigned.

Required Skills & Experience

Experience in rehab behavioral health or mental health preferred.

Outpatient clinic experience strongly considered.

Strong attention to detail especially with controlled substance handling.

Ability to deliver empathetic non-judgmental care to a diverse patient population.

Excellent communication reliability and teamwork skills.

Ability to multitask in a fast-paced outpatient environment.

Education & Licensure

Active LPN License in New York State required.

BLS certification preferred or required depending on site needs.

Schedule & Work Environment

Day shift 8 hours per day

MondayFriday with occasional Saturday (7 : 30am1 : 00pm)

Advanced notice of weekly site assignment (Ithaca or Cortland); no mid-week splitting when possible

Fast-paced outpatient environment serving patients who often utilize the clinic as primary care
