Position : Home Health Physical Therapist — Admissions / SOC
Schedule : PRN, Saturday & Sunday
Location : In-home visits (service area : KS / MO as assigned)
Compensation : $150 per Start of Care (SOC) (paid per visit)
Position Overview
We’re looking for a licensed Physical Therapist (PT) to complete weekend admissions / Start of Care (SOC) visits for Home Health patients. This PRN role is perfect for a PT who is confident with evaluations, enjoys independent work, and wants flexible weekend-only hours.
What You’ll Do
- Complete Start of Care (SOC) / admission PT evaluations in the home
- Perform comprehensive assessments (mobility, strength, balance, pain, safety / fall risk, functional status)
- Develop and document a skilled plan of care, goals, and visit frequency recommendations
- Provide patient / caregiver education and initiate home exercise and safety plans
- Communicate findings and recommendations with the care team and ordering providers as needed
- Ensure accurate, timely documentation per home health guidelines
