THE BASICS

Pay: You will earn a competitive salary of $51,000 - $55,000/year.

YOUR DAY AS A CASE MANAGER

As a Case Manager, each day will bring new and meaningful experiences. You'll dive into team meetings and Individual Service Plan (ISP) discussions, collaborating with colleagues to ensure our clients get the best support possible. You'll play a crucial role in greeting new clients during the admissions process and helping them transition smoothly into our services. Plus, you'll be involved in internal investigations, where your attention to detail and dedication to advocating for clients will shine. Your days will be filled with fulfilling work that truly aligns with our mission to uplift our community!

REQUIREMENTS FOR A CASE MANAGER

• Bachelor's degree in a related field of study
• 2+ years of case management experience in the Human Service field

Knowledge of the Department of Developmental Disabilities and Day Habilitation regulations, as well as CARF standards, is preferred.
