Responsibilities :
Assess patients' communication needs and swallowing needs, and develop individualized treatment plans
Provide therapy to improve speech, language, cognitive-communication, and swallowing skills
Collaborate with healthcare teams to ensure comprehensive patient care
Educate patients and families on communication strategies and safe swallowing techniques
Monitor and document patient progress and adjust treatment plans as needed
Implement interventions in various settings, including outpatient, acute care, and skilled nursing facilities
Participate in discharge planning and coordinate follow-up care for continued support
Qualifications :
A master's degree in Speech-Language Pathology from an accredited program
Active California licensure or certification as a Speech-Language Pathologist
Ability to work effectively in a Skilled Nursing Facility (SNF) setting
Strong communication, time management, and clinical decision-making skills
Preferred Skills and Qualifications
One or more years of experience in a Skilled Nursing Facility (SNF) setting
Familiarity with electronic documentation systems
Demonstrated commitment to professional growth and patient-centered care
